What is wrong (optional)

Thickness assumes the whole disc at the density of fully pressed KBr, 2.75 g/cm³. The figures used for comparison are those in the article: discs about 0.4 to 1 mm thick, about 0.2 to 1 % sample (a few percent for coarse grains), and about 430 to 740 MPa for the published methods that give a load and a die size; one further pressure, printed in an OCR scan, is not used. They show the spread of practice, not limits. The 13 mm die is listed as 13 mm (12.7 mm) and is calculated here at 13.0 mm; on a 12.7 mm bore the pressure is about 5 % higher. The tool does not predict whether a disc will be clear; that depends on drying, grinding and evacuation as well.
